About The Position

Well Care Health is seeking a passionate Home Health Physical Therapist to join their growing team and provide exceptional care in a supportive and rewarding environment. This role involves providing nursing care to patients in their homes, monitoring their conditions, educating patients and families, and collaborating with the healthcare team. The position covers Nash, Wilson, and Franklin Counties along the Hwy 64 Corridor and serves patients of all developmental stages throughout the lifespan, including adults and geriatrics. The role also involves participating in professional responsibilities such as program development, student programs, and performance improvement programs.

Requirements

  • Graduate of Physical Therapy program from an accredited university.
  • Licensure in the state in which providing therapy (NC/SC).
  • CPR certification.
  • Minimum 1 year of experience.
  • Knowledge of home health and physical therapy procedures.
  • Ability to manipulate patient care and evaluation equipment, including modalities and assistive devices.
  • Ability to properly transfer and guard patients.
  • Ability to apply other manual therapy techniques.
  • Ability to deal with the public, co-workers, patients, families, and physicians in a professional and positive manner.
  • Requires significant locomotion, frequent lifting greater than 50lbs and transferring patients greater than 100lbs.
  • Repeated reaching in front of the body, bending and stooping, pushing and pulling.
  • Ability to apply physical therapy clinical principles and use professional reasoning to develop appropriate treatment programs.
  • Ability to prioritize duties, learn new skills and techniques in patient care.
  • Ability to learn and use computer technology and supportive services.
  • Ability to see and communicate clearly in order to perform basic clinical techniques.
  • Must have a valid North Carolina driver’s license and an operational vehicle.

  • Works under the supervision of a Clinical Manager.
  • Provides individualized patient care for patients in all developmental stages throughout the life span including: Adult - 18-72 years, Geriatric - 72 + years, according to established policies procedures, and guidelines.
  • Occasionally requires reaching overhead, stair climbing, sitting and fine motor manipulation.
  • Exposure to Hazards: Noise, congestion, communicable disease, exposure to blood borne pathogens, patient assault, and injury resulting from improper body mechanics, uncooperative patients, families, and community hazards.
  • Population Served: Adolescents, adults, geriatrics, and pediatrics.

Responsibilities

  • Provide nursing care to patients in their homes, including administering medications, wound care, and other treatments.
  • Monitor patients’ conditions, report changes to the care team, and assist in developing personalized care plans.
  • Educate patients and families on health management, medications, and self-care techniques.
  • Collaborate with the healthcare team to ensure the highest quality care for each patient.
  • Assesses, interprets, plans, implements, and evaluates patients with appropriate level of skill given the patient’s age and diagnosis.
  • Admits patients to the agency when P.T. is a primary skilled need.
  • Assists with program development and discipline specific operations efficiently and productively.
  • Appropriately communicates and documents information in accordance to agency policies and procedures and discipline-specific guidelines.
  • Contributes to program effectiveness.
  • Organizes and performs work effectively and efficiently.
  • Maintains and adjusts schedule to enhance agency performance.
  • Demonstrates a daily commitment to the values of the agency.
  • Demonstrates positive interpersonal relations in dealing with all members of the agency.
  • Maintains and promotes customer satisfaction.
  • Effectively demonstrates the mission, vision, and values of the Agency on a daily basis.
  • Provides patient consultation and assumes communicative role with other professionals.
  • Refers patients to other professionals as appropriate and ordered.
  • Performs other duties as assigned.
