About The Position
As a Home Health Physical Therapist, you will provide individualized rehabilitation care directly in patients’ homes. You will evaluate patients, develop treatment plans, and guide them through recovery while helping them regain independence and prevent hospital readmissions. Home health allows physical therapists to work one-on-one with patients, build meaningful patient relationships, create flexible schedules, and focus on functional recovery in real-world environments.
Requirements
- Active Physical Therapist (PT) License in the state of practice
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- CPR / BLS Certification
- Valid driver's license and reliable transportation

- Familiarity with EMR documentation systems
- Strong communication and patient education skills
- Ability to work independently in the field
Responsibilities
- Perform comprehensive patient evaluations in the home
- Develop individualized treatment plans based on physician orders
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on home exercise programs
- Implement fall prevention and safety strategies
- Document patient progress in EMR systems
- Communicate with physicians, nurses, and interdisciplinary care teams
- Adjust treatment plans based on patient progress
