About The Position

In this role, you will provide home‑based therapy care to patients as directed by the attending physician. Requirements

  • Physical Therapist with a current license in the state of employment
  • Minimum of one (1) year of experience as a PT in an acute setting (required)
  • Ability to effectively communicate and create positive impressions with patients, families, physicians, and co‑workers
  • Knowledge of physical therapy best practices
  • Ability to make appropriate clinical judgments
  • Strong problem‑solving skills
  • Detail‑oriented and observant
  • Disciplined work ethic with the ability to prioritize and meet deadlines
  • Valid driver’s license and auto insurance with your name listed as a driver
  • Must meet all physical requirements of the position
  • Provide evidence of a current negative TB skin test or chest x‑ray at orientation
  • Annual TB testing required thereafter unless prior documentation is provided
  • Must be free of conditions that would restrict job performance
  • Position may involve exposure to bloodborne pathogens

Responsibilities

  • Provide skilled physical therapy services in the patient’s home as ordered by the attending physician
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and appropriate agency staff
  • Perform OASIS assessments to develop and adjust individualized plans of care
  • Complete initial evaluation and admission visits within 48–72 hours of referral
  • Develop and implement individualized plans of care
  • Help decrease re‑hospitalizations by front‑loading visits for high‑risk patients
  • Treat patients to relieve pain, restore function, and maintain maximum performance using physical therapy modalities such as exercise, massage, heat, and water
  • Arrange outpatient services when care cannot be safely provided in the home
  • Promote continuity of care through appropriate admissions, transfers, and discharges
  • Counsel patients and families on disease or injury management throughout all phases of therapy
  • Incorporate patients and families into plan‑of‑care development
  • Instruct patients and family members on the proper use of equipment (e.g., wheelchairs, braces, walkers, crutches, canes, prosthetics, orthotics)
  • Ensure patients and families understand financial responsibility for required equipment by coordinating with DME and/or MSW
  • Reduce re‑hospitalizations through patient education on preventative measures and informed decision‑making
  • Listen actively to patients and family members
  • Supervise PTAs and, when appropriate, nurse assistants
  • Evaluate and adjust plans of care based on patient response and medical condition
  • Communicate supervisory visit findings with PTAs and nurse aides
  • Complete all clinical documentation in accordance with agency protocols and Medicare/Federal guidelines
  • Document all treatments, assessments, and patient education
  • Complete chart audits at agency locations as requested
  • Maintain an active PT license in the state of employment
  • Communicate scheduling changes as needed
  • Participate in on‑call requirements and case conferences
  • Attend in‑service trainings, mandatory meetings, and orientation for new employees
  • Maintain a professional appearance as a company representative
  • Understand and adhere to agency policies and procedures
  • Drive to patient locations per scheduled visits
  • Maintain daily attendance at assigned visit locations
  • Document all subsequent, discharge, evaluation, recertification, and resumption‑of‑care visits within 24 hours
  • Document all start‑of‑care visits within 48 hours
  • Complete and submit all required documentation within company‑specified timelines
  • Establish and follow plans of care within the scope of practice for a Licensed Physical Therapist in the state of employment
