About The Position

This role involves delivering one-on-one physical therapy services in patients' homes, managing your own schedule with flexible weekly planning, and evaluating patients, completing start-of-care visits, reassessments, and discharges. The position offers zoned territories to minimize travel time and a stable patient census with strong long-term earning potential.

Requirements

• Active Florida Physical Therapist License REQUIRED
• Previous Home Health Experience REQUIRED
• Ability To Work Independently REQUIRED
• Strong Time Management And Organizational Skills REQUIRED

Responsibilities

• Deliver one-on-one physical therapy services in patients' homes
• Manage your own schedule with flexible weekly planning
• Evaluate patients, complete start-of-care visits, reassessments, and discharges
• Perform comprehensive home health physical therapy evaluations
• Complete start-of-care assessments, reassessments, and discharge visits
• Develop individualized treatment plans
• Deliver patient-centered rehabilitation services in the home
• Maintain accurate clinical documentation using WellSky/Kinser EMR
