About The Position

This position is for a Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the Canonsburg, PA area. The role involves delivering skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team. It requires performing comprehensive patient evaluations, functional mobility assessments, and developing individualized therapy plans to improve strength, balance, and independence. The therapist will also educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies. Responsibilities include critical thinking, problem-solving in real time, and ensuring thorough, compliant EMR documentation. The role requires working independently, traveling to patient homes within an assigned territory, and spending approximately 20% of the day driving.
Requirements
- Active Physical Therapist, PT license in the state you work
- One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
- Legally authorized to work in the United States
