About The Position

Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. This is a Home Health Physical Therapist (PT) position providing skilled care to patients in their homes in the Carbondale area. This position is eligible for $20,000 total bonus potential, including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $10,000 sign-on bonus paid within the first 18 months of employment. Bonus opportunities extend beyond the first year, with rewards increasing with tenure. The role involves delivering skilled, patient-centered care in a home health setting, performing comprehensive patient evaluations, functional mobility assessments, and developing individualized therapy plans. It requires critical thinking, problem-solving, and thorough EMR documentation. The position involves working independently while traveling to patient homes within an assigned territory.

Requirements

• Active Physical Therapist, PT license in the state you work.
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Successful completion of a drug screen prior to employment is part of our background process, which includes medical and recreational marijuana.
• Legally authorized to work in the United States.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team.
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.
