Join Elara Caring’s Home Health team in the Attleboro area and help patients regain strength, mobility and confidence in the place they feel most comfortable—their own homes. You’ll provide personalized care, manage your daily schedule in the field and collaborate with a supportive clinical team.
Requirements
- Degree from an American Physical Therapy Association-approved program
- Current, unrestricted Physical Therapist license in the state of practice
- Valid driver’s license, dependable transportation, and current auto insurance
- Ability and willingness to travel throughout Attleboro and surrounding communities
- Strong communication, organization, and independent decision-making skills
- Sit, stand, bend, lift, and move intermittently and be able to lift 50–100 lbs.
Responsibilities
- Evaluate patients’ strength, balance, mobility and functional abilities
- Create personalized treatment plans based on each patient’s needs and goals
- Provide therapy that helps patients move safely and remain independent at home
- Teach patients and caregivers about exercises, equipment and fall prevention
- Monitor progress and adjust treatment plans as needed
- Partner with physicians, nurses, therapists and other members of the care team
- Complete accurate and timely clinical documentation
- Provide guidance and clinical support to therapy staff when appropriate
