About The Position
Dependable Nursing Inc. is looking for a Part-Time Physical Therapist (PT) to provide skilled home health care to patients throughout East Phoenix and the East Valley. You'll provide personalized, one-on-one therapy while having greater control over your schedule.
Requirements
- Degree from an accredited Physical Therapy program
- Active Arizona Physical Therapist license
- Current CPR/BLS certification
- Excellent communication and time-management skills
- Ability to work independently in a home health environment
- Reliable transportation and valid driver's license
Nice To Haves
- Home health experience preferred
Responsibilities
- Complete in-home physical therapy evaluations
- Develop individualized treatment plans based on each patient's needs and goals
- Provide therapeutic exercise, gait training, balance training, mobility instruction, and other skilled interventions
- Help patients improve strength, function, endurance, and safety
- Educate patients and caregivers on home exercise programs and fall prevention
- Monitor patient progress and adjust treatment plans as appropriate
- Collaborate with nurses, physicians, therapists, and other members of the care team
- Complete accurate and timely documentation
