About The Position
Dependable Nursing Inc. is seeking a compassionate and motivated Physical Therapist (PT) to join our growing Home Health team serving South Phoenix and surrounding communities. This role offers the opportunity to provide personalized, one-on-one care in patients' homes, with the flexibility to choose between a full-time caseload or part-time hours. Therapists will have the independence of working in the field, supported by an experienced clinical team.
Requirements
- Degree from an accredited Physical Therapy program
- Active Arizona Physical Therapist license
- Current CPR/BLS certification
- Strong communication and organizational skills
- Ability to work independently and manage a field-based schedule
- Reliable transportation and valid driver's license
Nice To Haves
- Home health experience preferred
- Flexibility to travel outside your primary territory when needed to maintain a full-time caseload
Responsibilities
- Perform comprehensive physical therapy evaluations in patients' homes
- Develop individualized plans of care based on patient needs and goals
- Provide therapeutic exercises, mobility training, balance training, and other skilled PT interventions
- Help patients improve strength, function, safety, and independence
- Educate patients and caregivers on exercises, mobility techniques, and fall prevention
- Monitor progress and modify treatment plans as appropriate
- Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary team
- Complete accurate and timely clinical documentation
