About The Position

We are hiring a Full-Time Physical Therapist (PT) for a highly flexible Home Health position. This role offers one-on-one patient care, manageable territories, and strong earning potential—along with the ability to build your own schedule and work around your life.
Your Role:
As a Physical Therapist, you will evaluate, plan, and provide individualized treatment for patients in their homes while collaborating with a supportive clinical team.

Requirements

• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy
• Active Physical Therapy license in the state
• Valid driver’s license and reliable transportation

Responsibilities

• Provide high-quality patient care using approved therapy techniques
• Evaluate patients and establish clear, measurable goals
• Develop and implement personalized treatment plans
• Supervise and collaborate with PTAs and HHAs
• Document patient progress accurately and on time
• Educate patients and families on exercises, mobility, and equipment use
• Assess home environments and recommend safety modifications
• Coordinate care with nursing, social work, and other clinical staff
• Maintain HIPAA compliance and patient confidentiality
