About The Position

We’re hiring a Physical Therapist (PT) in Jacksonville, Florida who wants flexibility, autonomy, and strong earning potential in a home health setting. You’ll step into a clinical home health role focused on evaluations, start-of-care visits, reassessments, and discharges. You’ll manage your own schedule while delivering high-quality one-on-one care in patients’ homes across a zoned territory.

Requirements

• Licensed Physical Therapist in Florida
• Prior home health experience
• Comfortable working independently
• Value autonomy and productivity-based income

Responsibilities

• Conduct evaluations
• Perform start-of-care visits
• Conduct reassessments
• Manage discharges
• Deliver high-quality one-on-one care in patients’ homes
