Requirements
- Licensed Physical Therapist in Washington state (or before your first day of work)
- One to two years of experience as a Physical Therapist
- Current BLS certification
Responsibilities
- Provide comprehensive patient care through assessment, diagnosis, and treatment planning in home health settings.
- Develop and implement individualized care plans, focusing on rehabilitation and mobility enhancement.
- Educate patients and families on physical therapy techniques and home exercises.
- Collaborate with interdisciplinary teams to ensure continuity of care and effective discharge planning.
- Maintain accurate documentation of patient progress and treatment outcomes while adhering to medical terminology standards.
- Stay current with best practices in physical therapy, particularly in hospice and geriatrics.
