About The Position

We are seeking experienced and compassionate Physical Therapists (PTs) and Occupational Therapists (OTs) in the Tampa, Fl and surrounding areas to provide skilled care to patients in their homes. This role requires independence, strong clinical judgment, and proficiency in home health documentation and OASIS.

Requirements

• Active, unrestricted Florida PT or OT license
• Minimum 1 year recent Home Health experience
• Experience completing Start of Care (SOC), routine visits, and discharges in the home
• Hands-on experience with OASIS documentation
• Strong time management and documentation skills
• Reliable transportation and valid driver’s license
• Subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment

Responsibilities

• Perform Start of Care (SOC), recertifications, evaluations, and routine visits
• Develop and implement individualized care plans
• Complete accurate and timely OASIS documentation
• Educate patients and caregivers on treatment plans and safety
• Coordinate care with interdisciplinary team members
• Ensure compliance with all state and agency regulations
