Home Health Physical Therapist Assistant - PRN
About The Position
As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence in their homes. You will deliver personalized care focused on functional improvement and help patients overcome limitations and improve their quality of life. The role involves administering the treatment plan developed by a Physical Therapist, documenting care, coordinating with the clinical team, and applying infection-control practices. This is a patient-facing role subject to Humana’s TB screening program and driver safety requirements. The position is PRN, with one scheduled weekly hour listed.
Requirements
- Current state license to practice Physical Therapy.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population.
- Ability to read, write, and speak fluent English.

- One year of experience as a Physical Therapy Assistant.
Responsibilities
- Administer the Physical Therapist-developed treatment plan to patients in their homes.
- Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and heat, light, cold, water, and electrical modalities.
- Document client records and submit clinical documentation to the office on time each week.
- Attend educational meetings and coordinate department in-services.
- Notify the Physical Therapist or Branch Director about equipment failures, supply shortages, or changes involving staff or clients.
- Apply infection-control concepts and universal precautions while performing patient care activities.
