About The Position

Well Care Health is seeking a compassionate and dedicated Home Health Physical Therapist Assistant to join their growing team. This role involves providing exceptional care to patients in the comfort of their homes within a supportive and rewarding environment. The position works under the supervision of the Clinical Manager and licensed physical therapist, providing individualized patient care across all developmental stages (Adolescent, Adult, Geriatric). Responsibilities include administering prescribed physical therapy treatments, assisting with program development, documenting information, contributing to program effectiveness, organizing and performing work efficiently, maintaining and adjusting schedules, demonstrating commitment to agency values, fostering positive interpersonal relations, promoting customer satisfaction, and embodying the agency's mission, vision, and values.

Requirements

  • Associate Degree from an accredited physical therapy assistant program.
  • Licensure in the state in which providing therapy (NC/SC)
  • CPR certification.
  • Minimum 6 Months experience.
  • Knowledge of physical therapy procedures
  • Ability to manipulate patient care and evaluation equipment, including goniometers, modalities, assistive devices and exercise equipment
  • Ability to properly transfer and guard patients
  • Ability to apply other manual therapy techniques.
  • Ability to deal with the public, co-workers, patients, families, and physicians in a professional and positive manner.
  • Requires significant standing and walking, frequent lifting greater than 50 lbs and transferring patients greater than 100 lbs.
  • Repeated reaching in front of the body, bending and stooping.
  • Ability to apply physical therapy principles and use professional reasoning to develop appropriate treatment programs.
  • Ability to prioritize duties, learn new skills and techniques in patient care.
  • Ability to learn and use computer technology and supportive services.
  • Ability to see and communicate clearly in order to perform basic therapy techniques.
  • Must have a valid North Carolina driver’s license and an operational vehicle.

  • Works under the supervision of the Clinical Manager and licensed physical therapist.
  • Provides individualized patient care for patients in all developmental stages throughout the life span including: Adolescent - 13-17 years, Adult - 18-72 years, Geriatric - 72 + years, according to established policies, procedures, and guidelines as directed by the physical therapist.
  • Participates in professional responsibilities such as program development, student programs, and performance improvement programs.
  • Provides patient consultation and assumes a communicative role with other professionals.
  • Performs other duties as assigned.

Responsibilities

  • Provide nursing care to patients in their homes, including administering medications, wound care, and other treatments.
  • Monitor patients’ conditions, report changes to the care team, and assist in developing personalized care plans.
  • Educate patients and families on health management, medications, and self-care techniques.
  • Collaborate with the healthcare team to ensure the highest quality care for each patient.
  • Administers prescribed physical therapy treatments to patients in all stages of life.
  • Assists with program development and discipline specific operations efficiently and productively.
  • Appropriately communicates and documents information in accordance to agency policies and procedures and discipline specific guidelines.
  • Contributes to program effectiveness.
  • Organizes and performs work effectively and efficiently.
  • Maintains and adjusts schedule to enhance agency performance.
  • Demonstrates a daily commitment to the values of the agency.
  • Demonstrates positive interpersonal relations in dealing with all members of the agency.
  • Maintains and promotes customer satisfaction.
  • Effectively demonstrates the mission, vision and values of the Agency on a daily basis.
