About The Position

Concierge Home Care is seeking a Home Health Physical Therapist Assistant (PTA) to serve Tallahassee and surrounding areas. This role offers flexibility, independence, and the opportunity to increase earnings while providing meaningful one-on-one care in patients' homes. Requirements

• Active, unencumbered Florida Physical Therapist Assistant (PTA) license
• Valid driver’s license, reliable transportation, and auto insurance
• Employment is contingent upon successful completion of an AHCA Level 2 background screening.

• Home health experience preferred

Responsibilities

• Provide hands-on therapeutic care following the plan of care established by the Physical Therapist
• Assist patients in improving mobility, strength, and independence
• Collaborate with patients, caregivers, and interdisciplinary teams
• Monitor patient progress and communicate updates to the supervising PT
• Complete documentation using streamlined, AI-assisted tools
