About The Position

All About Home Care is a Houston-based home health care agency serving the Greater Houston area since 2004. We are currently seeking a Physical Therapist Assistant (PTA) to provide care to patients on the East side of the Greater Houston area (Northeast, Southeast, or Central East coverage). We are also looking for PRN PTAs for The Woodlands and Conroe areas. Our agency utilizes Kinnser software for patient care documentation. We are a skilled home health company providing a range of services including skilled nursing, physical therapy, occupational therapy, speech therapy, home health aides, and medical social workers. Our mission is to offer comfortable and dignified service through our professional and dedicated staff, catering to the specific needs of our patients.

Requirements

• Graduate from a two-year college level program approved by the American Physical Therapy Association
• Currently licensed in the state of Texas
• Two years of experience in Home Health preferred

Responsibilities

• Understands and adheres to established policies and procedures
• Provides physician prescribed physical therapy under a plan of care established by a PT
• Improves or minimizes residual physical disabilities of patients
• Returns the individual to optimum and productive level within patient’s capabilities
• Performs all skilled procedures as ordered by physician and according to the plan of care established by a PT
• Participates with all other health care personnel in patient care planning
• Takes initial history and perform initial evaluation
• Consults with PT regarding change in treatment
• Instructs patients and family/significant others in home programs and activities of daily living
• Participates in mandatory in-service programs and presents in-service programs as assigned
• Participates in performance improvement activities as assigned
• Attends all patient care conference as scheduled
• Prepares medical records, progress notes, and updates care plans for each patient visit in a timely manner as per Agency
