Requirements

Must hold valid MN PTA license
Able to travel around the Twin Cities Metro area to provide services to client's (we pay mileage!)
CPR/BLS Certification

Nice To Haves

Previous home health and/or outpatient experience strongly preferred

Responsibilities

Provide skilled physical therapy services in patients' homes by implementing treatment plans established by the supervising Physical Therapist, helping patients improve mobility, strength, balance, and independence.
Monitor patient progress and document care accurately and promptly, communicating changes in condition and collaborating with the interdisciplinary care team to ensure high-quality patient outcomes.
Educate patients on home exercise programs, safe mobility techniques, fall prevention, and the proper use of assistive devices to support continued recovery between visits.
