Requirements

  • Graduate of an occupational therapy school accredited by the Council of Medical Education of the AMA in collaboration with the American Association of Occupational Therapists and/or by the State Department of Education in the specific state where employed.
  • Registered by the Occupational Therapist Association and licensed in the state of practice if the state has registered practices.
  • Valid driver’s license and insured personal vehicle (per Lorian requirements).
  • Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
  • Possesses and maintains current CPR certification.

Responsibilities

  • Provide skilled occupational therapy services in accordance with physician orders and the patient’s individualized Plan of Care (POC).
  • Evaluate patients’ functional abilities and assist the physician and care team in determining appropriate treatment goals and interventions.
  • Contribute to the development and ongoing updates of the patient’s Plan of Care based on functional needs and progress.
  • Assess the home environment and recommend appropriate modifications, equipment, or strategies to promote safety, independence, and quality of life.
  • Monitor patient progress and communicate relevant changes and recommendations with the physician, case manager, patient, and family/caregivers.
  • Educate and collaborate with patients, families, and LORIAN team members regarding therapy programs, techniques, and strategies to support continued progress.
  • Maintain accurate, timely clinical documentation in accordance with LORIAN policies and procedures, while participating in continuing education and in-service programs.
