About The Position

Halcyon Home is hiring a Full Time Home Health Occupational Therapist to support our growing geriatric home health population in Kyle/Buda, TX and surrounding areas. Requirements

  • Bachelor of Science in Occupational Therapy.
  • Active state license as an occupational therapist or meets the requirements listed in 42 CFR §484.115(f). Military spouses with an equivalent out-of-state license are welcome.
  • Proficient therapy skills according to accepted standards of practice, good interpersonal skills, CPR certification, knowledge of durable medical equipment.
  • Reliable transportation, valid auto liability insurance, and current driver’s license.

Nice To Haves

  • A minimum of one year of home health experience preferred.
  • Familiarity with HomeCare HomeBase software is a plus.

Responsibilities

  • Perform ongoing clinical assessments to determine patient needs.
  • Evaluate patients’ occupational performance in self-care, work, leisure, and rest.
  • Assess and facilitate therapeutic interventions for ROM, strength, coordination, adaptive equipment, and functional mobility.
  • Develop and evaluate patient care plans in collaboration with patients, caregivers, and representatives.
  • Document clinical notes, phone orders, and other records promptly and accurately.
  • Provide education and counseling to patients and caregivers.
  • Supervise and guide occupational therapy assistants and home health aides as necessary.
  • Apply infection control and standard precautions during patient care activities.
  • Communicate effectively with physicians and healthcare practitioners involved in the patient’s care.
  • Meet continuing education requirements and demonstrate professional growth and competency.
  • Promote the Agency’s philosophy and policies to ensure high-quality care.
