About The Position

We are seeking a compassionate and motivated Occupational Therapist (OT) to join our growing Home Health team. The ideal candidate is patient-focused, organized, and passionate about helping individuals maintain independence and quality of life in their homes. Prudential Health Care was founded out of our love for helping others and our compassion to expand our Health Care expertise within the community. Our services include Home Health, Transitional Care, Private Duty Home Care and Hospice and Residential Care. Requirements

Current Occupational Therapist license in good standing
Graduate of an accredited OT program
Reliable transportation and valid driver’s license
Strong communication and time-management skills
Ability to work independently

Nice To Haves

Home health experience preferred (but not required)

Responsibilities

Evaluate patients in their homes and develop individualized treatment plans
Provide skilled OT services to improve daily living skills and functional independence
Educate patients and caregivers on adaptive techniques and safety
Document evaluations, progress notes, and discharge summaries in a timely manner
Collaborate with interdisciplinary healthcare team members
Follow all state, federal, and agency regulations
