About The Position

Acts is currently seeking qualified candidates for our Home Health Occupational Therapist (OT) per-diem position. In this role, you will be responsible for providing occupational therapy services to home health residents/clients in their home environment to promote functional independence, safety, and quality of life. Evaluates residents/clients, develops individualized plans of care, implements evidence-based interventions, and collaborates with the interdisciplinary care team to achieve optimal outcomes in compliance with physician orders, state practice acts, and federal and state home health regulations. Requirements

  • Bachelor's degree in Occupational Therapy from an accredited program
  • Minimum of 1-2 years of experience in home health is preferred
  • Valid Occupational Therapist license or eligibility for license in the state(s) where services are rendered
