About The Position

HealthPro Heritage at Home is hiring a Home Health Occupational Therapist to serve Waterford, Connecticut, and surrounding areas. The role provides home health services to adults and supports a growing partnership with Masonicare home health. This is a PRN opportunity with the ability to build your own schedule. Compensation is listed at a point base rate of $65–$75 per point. Requirements

Degree in Occupational Therapy from an accredited institution.
Valid state licensure as an Occupational Therapist, or eligibility for licensure.
Strong clinical assessment and diagnostic skills.
Proficiency in developing and implementing effective treatment plans.
Excellent communication and interpersonal skills with a patient-centered approach.
Ability to lift and assist patients as needed.
Ability to stand for extended periods and perform physical tasks related to therapy.

Responsibilities

Evaluate clients’ physical, emotional, and cognitive abilities using observation and standardized tools.
Develop personalized treatment plans to meet clients’ goals and improve their functional abilities.
Educate clients and their families about improving daily activities, including the use of adaptive equipment and environmental modifications.
Maintain accurate records of evaluations, progress, and treatment plans in compliance with legal and ethical standards.
Advocate for clients’ needs and support access to community resources, services, and accommodations.
