About The Position
HealthPro Heritage is seeking a Home Health Occupational Therapist to serve adults in Guilford, Connecticut, and surrounding areas. The role supports the agency’s growing partnership with Masonicare home health and offers flexible PRN scheduling. Compensation is a point-based rate of $65–$75 per point. The Occupational Therapist evaluates clients’ physical, emotional, and cognitive abilities; develops individualized treatment plans; and educates clients and families on daily activities, adaptive equipment, and environmental modifications. The role also includes maintaining compliant clinical documentation and advocating for clients’ needs and access to community resources and services.
Requirements
- Degree in Occupational Therapy from an accredited institution.
- Valid state licensure as an Occupational Therapist, or eligibility for licensure.
- Strong clinical assessment and diagnostic skills.
- Proficiency in developing and implementing effective treatment plans.
- Excellent communication and interpersonal skills with a patient-centered approach.
- Ability to lift and assist patients as needed.
- Ability to stand for extended periods and perform physical tasks related to therapy.
Responsibilities
- Evaluate clients’ physical, emotional, and cognitive abilities through observation and standardized tools.
- Create personalized treatment plans that address clients’ goals and improve their functional abilities.
- Educate clients and their families on improving daily activities, including the use of adaptive equipment and environmental modifications.
- Maintain accurate records of evaluations, progress, and treatment plans in compliance with legal and ethical standards.
- Advocate for clients’ needs and support access to community resources, services, and accommodations.
