About The Position

Well Care Health is seeking a passionate Home Health Occupational Therapist to join their growing team and provide exceptional care in a supportive and rewarding environment. This role involves providing individualized patient care for patients in all developmental stages throughout the lifespan in their homes. The position is responsible for the occupational therapy of patients in all stages of life in their home. The territory covered includes Franklin & Nash County. The role works under the supervision of a Clinical Team Leader.

Requirements

  • Graduate of Occupational Therapy program from an accredited university.
  • Licensure in the state in which providing services (NC/SC).
  • CPR certification.
  • Certification by AOTA.
  • Knowledge of home health and occupational therapy procedures.
  • Ability to manipulate patient care equipment, including modalities and assistive devices.
  • Ability to properly transfer and guard patients.
  • Ability to apply other manual therapy techniques.
  • Ability to deal with the public, co-workers, patients, families, and physicians in a professional and positive manner.
  • Requires significant locomotion, frequent lifting greater than 50 lbs and transferring patients greater than 100 lbs.
  • Repeated reaching in front of the body, bending and stooping, pushing and pulling.
  • Ability to apply rehabilitation clinical principles and use professional reasoning to develop appropriate treatment programs.
  • Ability to prioritize duties, learn new skills and techniques in patient care.
  • Ability to learn and use computer technology and supportive services.
  • Ability to see and communicate clearly in order to perform basic clinical techniques.
  • Must have a valid North Carolina Driver’s license and an operational vehicle.

Responsibilities

  • Assesses, interprets, plans, implements, and evaluates patients with appropriate level of skill according to the patient’s age and diagnosis.
  • Assists with program development and discipline specific operations efficiently and productively.
  • Appropriately communicates information in accordance with agency policies and procedures and discipline-specific guidelines.
  • Contributes to program effectiveness.
  • Organizes and performs work effectively and efficiently.
  • Maintains and adjusts schedule to enhance agency performance.

  • Provides nursing care to patients in their homes, including administering medications, wound care, and other treatments.
  • Monitors patients’ conditions, report changes to the care team, and assist in developing personalized care plans.
  • Educates patients and families on health management, medications, and self-care techniques.
  • Collaborates with the healthcare team to ensure the highest quality care for each patient.
  • Performs patient evaluations and treatments according to procedural guidelines for home health patients referred to Occupational Therapy.
  • Participates in professional responsibilities such as program development, student programs, and performance improvement programs.
  • Provides patient consultation and communicates with other professionals.
  • Performs other appropriate duties as assigned.
