The Home Health Occupational Therapist (OT) will perform ongoing accurate and comprehensive assessments of each patient, accurately complete OASIS data sets, and obtain necessary physician/allowed provider orders. The role involves collaborating with physicians/allowed providers as patient needs dictate, developing and implementing written plans of care, and ensuring changes in patients’ clinical status are communicated to the healthcare team. The OT will coordinate care planning and delivery, educate patients, caregivers, families, and staff on therapy modalities, and develop appropriate home exercise programs. They will deliver occupational therapy modalities in accordance with the standard of care, provide care to patients on caseload, and maintain clear, concise documentation. The position requires providing, modifying, or discontinuing therapy modalities based on ongoing assessment and physician orders, assessing and reassessing pain, and utilizing appropriate pain management modalities. The OT will provide modalities in an environment that optimizes patient safety and reduces medical errors, participate in case conferences, and perform transfer and discharge planning functions. Recommendations for needed referrals to other disciplines and community resources will be made, and necessary supplies and equipment will be obtained. The role involves developing, planning, organizing, and directing patient care services in accordance with regulations and standards, assessing patients for durable medical equipment and adaptive equipment needs, and supporting cultural practices that do not harm the patient. The OT will provide supervision and mentorship to OT assistants, including supervisory visits, and identify and mitigate environmental risks to patient safety. Maintaining current knowledge of community resources and accessing appropriate resources to meet patient needs is also a key responsibility.
