About The Position

CenterWell Home Health is seeking a Home Health Occupational Therapist Assistant to help patients regain strength, mobility, independence, and quality of life in their homes. The assistant delivers personalized rehabilitation and functional improvement services planned, delegated, and supervised by a qualified Occupational Therapist, in accordance with each patient's Plan of Treatment and physician orders. The role also includes documenting care, coordinating with the interdisciplinary team, instructing patients and families, and supporting discharge planning.

Requirements

• Current and unrestricted OTA licensure.
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.

Nice To Haves

• At least six months of occupational therapist assistant experience.
• Home Health experience.

Responsibilities

• Provide therapy services planned, delegated, and supervised by a qualified Occupational Therapist in accordance with the patient's Plan of Treatment.
• Assist with vocational and educational programs and activities established by a registered Occupational Therapist to restore, reinforce, and enhance task performance, address pathology, and promote or maintain health and self-sufficiency.
• Design or adapt equipment and working or living environments.
• Fabricate devices to improve function, independence, and participation in programs or the community where possible.
• Provide therapeutic treatment and instruction as directed by the qualified Occupational Therapist and in accordance with physician orders to improve or restore strength, coordination, range of motion, and function, or teach compensatory measures.
• Report information and observations to the Occupational Therapist and/or Clinical Supervisor, document observed information in patient records, and prepare clinical notes.
• Assist with preparation of progress reports.
• Maintain and submit documentation as required by the Company and/or facility.
• Instruct patients and family members on home programs and the care and use of adaptive equipment.
• Participate in care coordination and discharge planning, and serve as a resource to other healthcare personnel in meeting patient needs.
• Design community reintegration activities, as appropriate, to support physical reconditioning and/or psychological adjustment, and coordinate plans with the interdisciplinary team.
