Requirements

Active RN license in the state of Virginia
Reliable transportation and valid driver's license for travel throughout the NOVA region
Ability to work independently and manage a flexible patient caseload
Compassionate, patient-centered approach to care
Commitment to working a minimum of 2 days a week, with one day being a Monday and the other a Wednesday or Friday.

Nice To Haves

Previous home health, hospital med-surg, or community nursing experience preferred
Strong clinical assessment and critical thinking skills
Excellent communication and patient education abilities

Responsibilities

Provide skilled nursing care to patients in their homes across the Northern Virginia area
Conduct comprehensive patient assessments and develop individualized care plans
Administer medications, wound care (wet and dry), wound vac, IV therapy, ostomy care and other clinical treatments as ordered.
Monitor and document patient progress, vital signs, and changes in condition
Educate patients and family members on disease management, medications, and home care techniques
Collaborate with physicians, therapists, and care team members to ensure coordinated patient care
Maintain accurate and timely clinical documentation in compliance with regulatory standards
