Home Health Nurse- LPN/ RN
About The Position
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting. We have multiple client opportunities in Ypsilanti, MI, offering various schedules including day, night, and weekend shifts. These roles involve caring for adult and pediatric clients with complex medical needs, including ventilator, tracheostomy, G-tube, and central line management. Supportive families are on-site to assist and provide orientation. Some homes have pets.
Requirements
- Active RN or LPN license in the state of Michigan
- Strong communication and critical thinking skills
Nice To Haves
- Home care or private duty experience preferred
- Trach/vent experience a plus (training provided)
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care)
- Monitor and document patient condition and vital signs
- Assist with daily living activities (ADLs)
- Communicate with families and care teams
- Maintain safe, high-quality, patient-centered care
