Home Health Nurse- LPN/ RN
About The Position
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting. This specific client is a 3-month-old male in Holly who was born with congenital birth defects requiring high-level care to manage floppy, easily obstructed airways and safe feeding via alternative methods. He will require trach management as well as oxygen management as needed, and nutrition via G-tube. The household includes his parents, a 3-year-old sibling, and one friendly dog. Open shifts are available for the Night Shift: 7:00 pm - 7:00 am.
Requirements
- Active RN or LPN license in the state of Michigan
- Strong communication and critical thinking skills
Nice To Haves
- Home care or private duty experience preferred
- Trach/vent experience a plus (training provided)
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care)
- Monitor and document patient condition and vital signs
- Assist with daily living activities (ADLs)
- Communicate with families and care teams
- Maintain safe, high-quality, patient-centered care
