3HC Home Health and Hospice is seeking a compassionate Home Health Nurse, RN to provide skilled nursing care to patients requiring intermittent professional nursing services. Requirements
- Registered Nurse currently licensed to practice nursing in the state of North Carolina.
- Valid North Carolina Driver's License.
Nice To Haves
- Minimum of one-year experience as a professional nurse in an acute care setting.
- Minimum of one-year Home Health experience.
Responsibilities
- Administer skilled nursing care to patients requiring intermittent professional nursing service.
- Abide by and support 3HC's Compliance Program and Code of Ethics.
- Create positive experiences for internal and external customers, displaying courtesy, tact, and knowledge of agency services.
- Manage assigned cases and assist the office in achieving positive patient outcomes, strategic plan, and budget.
- Conduct a complete assessment of the patient to identify appropriate care needs.
- Document in the medical record electronically and download daily.
- Complete OASIS at specific points in time (admission, resumption of care, recertification, etc.).
- Provide regular supervision to Licensed Practical Nurses and paraprofessionals according to regulatory guidelines.
- Maintain regular communication with supervisor and communicate patient problems on a timely basis following the chain of command.
- Ensure that 3HC can bill for services appropriately.
- Demonstrate cost-effectiveness in the use of agency resources, monitoring waste, and proper use of supplies and equipment.
