Summary: The Home Health Nurse, RN is responsible for administering skilled nursing care to patients requiring intermittent professional nursing service.

Requirements

Registered Nurse currently licensed to practice nursing in the state of North Carolina
Minimum of one-year experience as a professional nurse in an acute care setting
Maintain current CPR certification (BCLS)
Basic computer skills/knowledge and experience in electronic medical records
Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public
Must be competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the patient's individualized a plan of care
Possess valid Driver's License and reliable transportation

Responsibilities

Administering skilled nursing care to patients requiring intermittent professional nursing service.
Abides by and supports 3HC's Compliance Program and Code of Ethics.
Creates positive experiences for internal and external customers.
Manages assigned cases and assists office with achieving positive patient outcomes, strategic plan and budget.
Conducts a complete assessment of the patient to identify appropriate care needs.
Documents in the medical record electronically and downloads daily.
Ensures that the physician is involved ongoing with the patient's care.
Completes OASIS at specific point in time, such as admission, resumption of care, recertification, etc.
Provides regular supervision to the Licensed Practical Nurses and paraprofessionals who are providing services to the patient according to regulatory guidelines, CMS/Hospice guidelines.
Collaborates with supervisor to meet assigned productivity of 100%.
Rotates weekday, weekend, and holiday "on-call" and "back-up-call".
Maintains regular communication with supervisor.
Ensures that 3HC can bill for services appropriately.
Travels to other offices to meet the demand of patient needs as requested by the Director of Clinical Practice/Assistant Director of Clinical Practice.
Responsible for working from 5pm Friday - 8am Monday.
Provide specialized hospice training to other staff, family members and informal caregivers to insure adequate care.
Develops and implements the patient care plan in cooperation with other agency professional and paraprofessionals who will be providing care to the patient in NetSmart.
Ensures complete documentation on an ongoing basis supporting hospice guidelines and COPs.
Turns in all paperwork to designated person in the office by 5:00pm on Monday and communicates laptop daily Saturday and Sunday.
Makes verbal/written report of new admissions and ER visits to primary nurse Sunday night or Monday morning between 8am-9am.
Completes scheduled visits after 5pm on Fridays as warranted.
Works at least one weekend day.
Collaborate with supervisor to meet productivity of 100% for work hours 12pm-11pm Monday-Thursday.
Covers on-call for clinical office Monday-Thursday.
Visits done after 11pm will be paid at over quota rate and not considered part of quota.
Stays abreast of overall patient care and improves nursing care skills.
Demonstrates an interest in personal and professional growth for self and staff.
Demonstrates a willingness to be cost effective in the use of agency resources, the monitoring of waste, and the proper and safe use of supplies and equipment.
Adheres to 3HC's Personnel Policy and performs other appropriate duties as assigned by supervisor to promote the successful operations and future growth of 3HC.
