As an RN Case Manager, you'll coordinate, manage, and deliver individualized care to patients in their homes while serving as a key member of an interdisciplinary healthcare team.

Requirements

Current Registered Nurse (RN) license in Iowa, in good standing.
Graduate of an accredited nursing program.
Valid driver's license, reliable transportation, and proof of automobile insurance that meets VNA requirements.
Ability to travel throughout the service area.
Strong clinical assessment, communication, organization, and critical thinking skills.
Excellent communication, organization, and time management abilities.
Ability to work independently while collaborating effectively with physicians, patients, families, and interdisciplinary team members.
Ability to work independently while collaborating effectively with an interdisciplinary team.
All offers of employment are contingent upon the successful completion of a criminal background check conducted by the Iowa Division of Criminal Investigation (DCI) and verification of eligibility to work in healthcare programs, including a negative finding on the Office of Inspector General (OIG) Excluded Provider List.

Nice To Haves

One year of clinical nursing experience preferred
home health, hospice, case management, community health, or public health nursing experience strongly preferred.
Case management experience.
Knowledge of Medicare home health regulations and documentation requirements.
Experience utilizing electronic medical records.
OASIS assessment experience.

Responsibilities

Develop, implement, and manage individualized patient care plans.
Complete comprehensive nursing assessments and ongoing patient evaluations.
Provide skilled nursing care, treatments, and disease management education.
Monitor patient progress and adjust care plans to promote positive outcomes.
Coordinate services with physicians, therapists, social workers, and other healthcare professionals.
Educate patients and caregivers regarding diagnoses, medications, treatments, and self-management strategies.
Identify changes in patient condition and appropriately communicate recommendations to the care team.
Ensure compliance with Medicare, state, and organizational documentation standards.
Maintain accurate, timely electronic documentation.
Build strong, trusting relationships with patients and families while advocating for their healthcare needs.
Participate in care conferences, quality improvement activities, and interdisciplinary collaboration.
