Requirements

Master’s Degree in Social Work (required)
Minimum one year MSW experience within the last three years
Flexibility to meet patient scheduling needs, including new admissions

Nice To Haves

Home health or hospice experience preferred
Experience with geriatric population preferred

Responsibilities

Develop and implement individualized MSW plans of care in collaboration with physicians and the interdisciplinary team
Assess psychosocial needs and determine eligibility for home health services
Provide counseling, resource coordination, and crisis intervention
Re-evaluate and adjust care plans as patient needs change
Support patients and families navigating complex healthcare decisions
