About The Position
Our client is seeking a compassionate Home Health Medical Social Worker with a Master’s in Social Work and relevant experience to join their care team. The role involves collaborating to provide patient care plans and supporting patients in the Lansing, MI area. The position is part-time and requires travel to patient locations.
Requirements
- Master's degree or higher in Social Work from graduate school accredited by the Council of Social Work.
- One (1) to two (2) years post graduate social work experience in a health care setting.
- Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work.
- Current driver’s license
- Reliable Mode of Transportation
- Auto Insurance
- Ability to spend ~20% of your day driving to/from patient locations
- Experience with Electronic Medical Record (EMR) software
- Experience with Other EMR software
- Experience with TruChart EHR software
Responsibilities
- Collaborate to provide management and delivery of patient care plans
- Support patients in Jackson and surrounding areas
- Meet critical deadlines for charting
- Self-manage time and schedule
- Use tablets, mobile phones and EMR software
