About The Position

Provide coordinated care in the home to clients of all age groups and perform psychosocial assessments, analysis, counseling and referrals to meet the needs of the client/family. This role involves performing an initial evaluation during the admission process, assessing psychosocial, financial, environmental, and community needs. The Medical Social Worker will maintain a working knowledge of public and private eligibility standards and requirements, collaborate with an interdisciplinary team, and accept clinical assignments consistent with education and competence. Effective communication with clients, families, team members, and other healthcare professionals is essential, as is regular participation in Case Conferences/Team Meetings. The role also requires demonstrating commitment and professional growth through in-service programs and meeting mandatory continuing education requirements.

Requirements

  • 1 year of experience as an MSW in a health care setting.
  • MSW skills as defined by generally accepted standards of practice.
  • Excellent interpersonal skills and ability to communicate effectively.
  • Proven decision-making skills.
  • If required to make visits, Hepatitis profile.
  • Must read, write, and comprehend English.
  • Bachelors or Master's degree in Social Work.
  • Current driver's license in good standing.
  • Reliable transportation and auto liability insurance.
  • Current MSW Certification that is in good standing to practice as an MSW in any/all applicable states.
  • Current CPR certificate.
  • On-going employment as an MSW always requires the maintenance of a valid MSW Certification in the state where the employee practices and a valid CPR certificate.

Responsibilities

  • Perform an initial evaluation during the admission process, assessing the client’s psychosocial, financial, environmental and community needs.
  • Maintain a working knowledge of public and private eligibility standards and requirements.
  • Collaborate with interdisciplinary team to promote coordination of patient care.
  • Accept clinical assignments that are consistent with education and competence to meet the needs of the patients.
  • Provide effective communication to clients, their families, team members, and other health care professionals to achieve desirable outcomes.
  • Regular participation in Case Conference/Team Meetings.
  • Participate in in-service programs and maintain/improve competency.
  • Meet mandatory continuing education requirements of the Agency/licensing board.
