About The Position
We are seeking a compassionate, dependable, and experienced Licensed Vocational Nurse (LVN) to provide high-quality home health care to patients throughout our Texas service area. Join our growing home health team!
Requirements
- Current Texas LVN license in good standing
- Minimum 1 year of recent home health experience required
- Strong clinical assessment and documentation skills
- Excellent communication and time management skills
- Ability to work independently while providing compassionate patient care
- Current CPR certification
- Reliable transportation, valid driver's license, and current auto insurance required
Responsibilities
- Perform skilled nursing visits in patients' homes
- Conduct thorough patient assessments and monitor changes in condition
- Develop and implement individualized plans of care under the supervision of the RN
- Accurately complete electronic documentation in a timely manner
- Educate patients and caregivers on disease management, medications, and home safety
- Communicate effectively with physicians and the interdisciplinary care team
- Ensure compliance with agency policies and Medicare guidelines
