About The Position

Halcyon Home is hiring a Home Health LVN PRN - Per Visit to support our growing geriatric home health population in Irving/Arlington/Eastern Tarrant Country, TX and surrounding areas. The ideal candidate is a charismatic communicator who has the gift of reading people, loves engaging with people, and takes pride in getting everyone taken care of. A dedicated LVN with home health experience (or experience in a related field and a desire to learn about home healthcare) who is a team player and thrives in a supportive environment is encouraged to apply.

Requirements

  • Graduate of an accredited school of vocational/practical nursing.
  • Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact.
  • One year of experience as a licensed vocational/practical nurse in a clinical care setting required.
  • Nursing skills as defined as generally accepted standards of practice.
  • Good interpersonal skills.
  • Proof of current CPR certification.
  • Reliable transportation and valid auto liability insurance.

Responsibilities

  • Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation.
  • Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN.
  • Provides services that are ordered by the physician as indicated in the plan of care.
  • Provides patient and caregiver education.
  • Provides care using infection control measures that protect both staff and patient.
  • Assures the continuity of care through delivery of quality patient care.
  • Provides effective communication to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes.
  • Completes appropriate documentation in a timely manner.
  • Monitors assigned cases to ensure compliance with requirements of third-party payor.
  • Participates in the Agency’s QAPI program.
  • Participates in the Agency sponsored in-service trainings.
  • Demonstrates commitment, professional growth, and competency.
  • Promotes the Agency philosophy and administrative policies to ensure quality of care.
