About The Position
Now interviewing for 1 LPN to work in our upper Vermont territory! This position involves providing home health and hospice care, requiring travel from patient to patient for home visits. The ideal candidate must be able to manage and adjust to changes in schedule and possess a valid driver's license in good standing. The EMR system used is HealthWyse (Careficient). This is an outpatient care role.
Requirements
- Home Health and Hospice Experience required
- Experience traveling from patient to patient for home visits
- Must be able to manage and adjust to changes in schedule
- Must have drivers license in good standing
- BLS required
- Familiarity with HealthWyse (Careficient) EMR system is a plus.
- Outpatient care experience
- Ability to perform wound care.
