About The Position

Sage Group HealthCare Staffing is seeking a Home Health LPN for a 13-week travel/contract position in Lacey, WA. This role involves providing skilled nursing care under RN supervision, monitoring patient conditions, administering medication, educating patients and families, and completing clinical documentation. The position requires covering Lewis county with potential crossover into other areas, necessitating flexibility.

Requirements

• LPN experience: 1 - 2 years
• Recent/Current LPN Home Health experience: 1 year within the last 3 years
• Home Care Home Base (HCHB) charting
• Valid Driver’s License
• Personal Auto Insurance
• LPN License for State of the state applying or Compact if acceptable
• BLS Certification from AHA or American Red Cross

• Must be flexible with areas of coverage if crossover is required.

Responsibilities

• Provide skilled nursing care as ordered by the attending physician under RN's supervision.
• Observe and monitor patient conditions and notify RN case manager of any changes.
• Administer medication as prescribed by the physician under RN's supervision.
• Educate the patient on preventative measures and decision-making to help decrease re-hospitalizations.
• Educate the patient and family on the disease process using teach back methods to ensure understanding.
• Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Understand and follow agency policies, procedures, rules, and regulations.
• Communicate changes in schedule/availability to schedulers or supervisors.
• Attend IDT Team meetings.
