About The Position

Licensed Practical Nurses (LPN) among the Franciscan team are those who choose to focus on home healthcare are a special group of people that can provide direct patient care. In addition, this role holds the responsibility of educating caregivers and actively participating on the patient’s care team. This is a level of nursing that must be able to accurately assess needs, provide appropriate interventions, and revise care as needed under the supervision of a registered nurse. Requirements

  • Associate's Degree or Diploma from Accredited Licensed Practical Nurse Program - Required
  • 3 Years Acute Care or Long-Term Care Experience - Required
  • Licensed Practical Nurse (LPN)- State Licensing Board - Required
  • Cardiopulmonary Resuscitation (CPR)- American Heart Association- Required
  • Driver's License - Required
  • Auto Insurance - Required

Nice To Haves

  • 1 year Home Health Experience - Preferred

Responsibilities

  • Implement nursing plan of care at the direction of the registered nurse.
  • Provide home-based patient care based on physician orders and the established nursing plan of care.
  • Collaborate with Home Health RN and multidisciplinary treatment team to individualize each patient’s treatment needs, to achieve defined goals for improved patient outcomes.
  • Conduct patient psychosocial assessments, to explore the physical, psychological, and social aspects of the patient and their conditions, obtain history, and elicit an accurate description of psychosocial needs.
  • Contribute to developing discharge plans, identifying the post-discharge needs of the patient, and target appropriate resources for follow-up care, in collaboration with the patient, family and care team.
  • Document patient care information, and ensure timely and accurate completion of patient care records in accordance with policies and procedures.
