About The Position

Provide direct, skilled nursing care in patients’ homes under the supervision of an RN, following physician-ordered care plans. Perform clinical treatments including wound care, medication administration, injections, IV therapy, and vital signs monitoring. Observe and assess patient conditions, promptly reporting any changes to the RN or physician. Collaborate with RN Case Managers and interdisciplinary teams to support and update individualized care plans. Educate patients and families on disease management, medications, safety, and self-care techniques. Supervise and support Home Health Aides, ensuring quality and consistency of care. Maintain accurate, timely clinical documentation in compliance with agency, state, and payer requirements. Participate in case conferences and quality improvement initiatives. Uphold strict infection control and safety standards during all patient interactions. Efficiently manage daily travel and visit schedules within your assigned territory.

Requirements

  • At least 1 year of clinical nursing experience and 1 year of home health experience
  • Strong ability to follow physician-directed treatment plans and maintain thorough, accurate medical records
  • Excellent communication and interpersonal skills with a patient-first mindset
  • Ability to work independently while collaborating effectively with a care team
  • Graduate of an accredited Practical or Vocational Nursing program
  • Active LPN/LVN license in the state of practice
  • Current CPR certification (must be maintained throughout employment)
  • Valid driver’s license and reliable transportation to travel between patient homes

Benefits

  • Salary: $70,000
  • Schedule: Monday–Friday, 8:00 AM – 5:00 PM
