About The Position

The Home Health LPN provides direct, skilled nursing care to patients in their homes under the supervision of a Registered Nurse, adhering to physician-ordered care plans. This role involves performing various clinical treatments such as wound care, medication administration, injections, IV therapy, and vital signs monitoring. The LPN is responsible for observing and assessing patient conditions, reporting changes to the RN or physician, and collaborating with interdisciplinary teams to update care plans. Key aspects include educating patients and families on disease management and self-care, supervising Home Health Aides, maintaining accurate clinical documentation, participating in quality improvement, upholding infection control standards, and managing daily travel schedules within an assigned territory.

Requirements

  • At least 1 year of clinical nursing experience
  • 1 year of home health experience
  • Strong ability to follow physician-directed treatment plans and maintain thorough, accurate medical records
  • Excellent communication and interpersonal skills with a patient-first mindset
  • Ability to work independently while collaborating effectively with a care team
  • Graduate of an accredited Practical or Vocational Nursing program
  • Active LPN/LVN license in the state of practice
  • Current CPR certification (must be maintained throughout employment)
  • Valid driver’s license and reliable transportation to travel between patient homes

Responsibilities

  • Provide direct, skilled nursing care in patients’ homes under the supervision of an RN, following physician-ordered care plans
  • Perform clinical treatments including wound care, medication administration, injections, IV therapy, and vital signs monitoring
  • Observe and assess patient conditions, promptly reporting any changes to the RN or physician
  • Collaborate with RN Case Managers and interdisciplinary teams to support and update individualized care plans
  • Educate patients and families on disease management, medications, safety, and self-care techniques
  • Supervise and support Home Health Aides, ensuring quality and consistency of care
  • Maintain accurate, timely clinical documentation in compliance with agency, state, and payer requirements
  • Participate in case conferences and quality improvement initiatives
  • Uphold strict infection control and safety standards during all patient interactions
  • Efficiently manage daily travel and visit schedules within your assigned territory
