Requirements
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current State License as a Licensed Practical Nurse (LPN/LVN)
- At least 1 year of clinical experience
- Strong communication skills and a team-first mindset
- Ability to work independently while staying closely connected to your RN partner
- Valid driver’s license, insurance, and reliable transportation
- Willingness to travel within the assigned territory
Nice To Haves
- Home health or hospice experience preferred, but not required
Responsibilities
- Partner closely with your RN Case Manager to deliver care aligned with each patient’s individualized Plan of Care
- Provide skilled nursing visits, reinforcing care plans and patient goals in the home setting
- Communicate changes in condition, concerns, and updates to your RN partner to ensure coordinated, proactive care
- Contribute your clinical insight—your observations help shape and improve the Plan of Care
- Educate patients and caregivers on medications, treatments, and self-care techniques
- Help prevent avoidable hospitalizations through early intervention and patient support
- Maintain accurate, timely documentation of visits and care provided
- Build trust with patients and families through compassionate, respectful care
