Requirements

  • Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
  • Current State License as a Licensed Practical Nurse (LPN/LVN)
  • At least 1 year of clinical experience
  • Strong communication skills and a team-first mindset
  • Ability to work independently while staying closely connected to your RN partner
  • Valid driver’s license, insurance, and reliable transportation
  • Willingness to travel within the assigned territory

Nice To Haves

  • Home health or hospice experience preferred, but not required

Responsibilities

  • Partner closely with your RN Case Manager to deliver care aligned with each patient’s individualized Plan of Care
  • Provide skilled nursing visits, reinforcing care plans and patient goals in the home setting
  • Communicate changes in condition, concerns, and updates to your RN partner to ensure coordinated, proactive care
  • Contribute your clinical insight—your observations help shape and improve the Plan of Care
  • Educate patients and caregivers on medications, treatments, and self-care techniques
  • Help prevent avoidable hospitalizations through early intervention and patient support
  • Maintain accurate, timely documentation of visits and care provided
  • Build trust with patients and families through compassionate, respectful care
