Requirements
- Current Washington State LPN license (or eligible before your first day)
- Prior home health or hospice experience preferred
- Confidence to work independently and apply strong clinical judgment
- Current BLS certification from AHA
- Valid driver’s license and reliable, insured vehicle
Responsibilities
- Provide one-on-one skilled nursing care in patients’ homes (e.g., wound care, catheter care, medication administration, symptom management)
- Conduct skilled observation, assessment, and evaluation
- Educate and support patients, families, and caregivers
- Collaborate with healthcare providers and the interdisciplinary team
- See an average of 5–6 patients per day
