Requirements

Active LPN license in the state providing patient care
Graduate of an accredited practical nurse or vocational nursing program
Maintain current CPR certification
Two (2) years of nursing experience in Community/home health or medical/surgical experience preferred

Responsibilities

Follow plans of care
Provide treatments
Collaborate with members of the team to meet positive patient care outcomes
Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST) to the RN and/or Clinical Manager
Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care
Provide direct patient care
Implement plan of care initiated by the RN
Provide accurate and timely documentation
Use equipment and supplies effectively and efficiently
Participate in personal and professional growth and development
