About The Position

This role involves providing direct client care within the scope of licensed vocational nursing practice. The position requires implementing care plans, conducting ongoing assessments, and providing education and emotional support to clients and their families. Accurate documentation, medication administration, and emergency response are key aspects of this role. The nurse will also participate in coordinating home health services and adhere to agency policies and infection control principles.

  • Competent performance of skills, treatments and procedures
  • Adherence to agency policies, procedures and protocols
  • Knowledge of infection control and Universal Precautions
  • Ability to identify and participate in performance improvement activities
  • Proper and safe use of equipment

Responsibilities

  • Provides direct client care as defined under the scope of nursing practice and the legislative acts usingsound judgment and critical thinking.
  • Implements plan of care and treatment as required.
  • Provides accurate, detailed and timely documentation consistent with the plan of care and the nursingprocess.
  • Conducts ongoing assessments.
  • Participates in health/therapeutic counseling, teaching and emotional support to the client and family/caregivers.
  • Observes client condition and accurately reports to the registered nurse.
  • Assists with client discharge or transfer.
  • Assesses and provides client and family/caregiver education and information pertinent to diagnosis.
  • Prepares, administers, and appropriately documents actions taken specific to commonly prescribed oral, topical, subcutaneous and intramuscular medications. Observes, reports and records adverse reactions to medications and/or treatment to the registered nurse.
  • Recognizes the need for and institute emergency measures in a rapid and appropriate manner, when indicated.
  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (i.e., PT, O, ST, Dietician) to the Care Designer/Director of Nursing or registered nurse supervisor.
  • Performs all skills, treatments and procedures competently in accordance with State Board of Nursing scope of practice Nursing Practice and adheres to the policies, procedures and protocols of the agency.
  • Follows principles of infection control and Universal Precautions.
  • Identifies and participates in performance improvement activities.
  • Uses equipment properly and in a safe manner.
