About The Position
This role involves providing direct skilled nursing care to patients in their homes. Key responsibilities include performing various nursing procedures, monitoring patient conditions, educating patients and families, and maintaining accurate documentation. The position requires travel within an assigned territory.
Requirements
- Current California LVN license in good standing
- At least 1 year of nursing experience
- Valid CA driver’s license, reliable transportation, auto insurance
- CPR certification
- Strong communication skills
- Ability to work independently; comfortable with home visits
Nice To Haves
- Home health experience preferred
- Bilingual skills a plus (e.g. Spanish, Vietnamese, etc.)
Responsibilities
- Provide direct skilled nursing care according to physician’s orders and the plan of care
- Perform wound care, injections, medication administration, IV support (if applicable)
- Monitor patient vitals, document assessments, progress notes, and coordinate with case managers / supervisors
- Educate patients and families / caregivers on care plans, disease processes, home safety, etc.
- Travel to patient homes within assigned territory; maintain charting & documentation accurately and timely
