About The Position

We’re hiring a Home Health Licensed Vocational Nurse (LVN) who wants stability, autonomy in the field, and the opportunity to provide meaningful care in patients’ homes. You’ll step into a full-time home health role focused on managing a growing patient census while providing direct nursing care and case management support. You will complete assessments and documentation within LVN scope, assist with admissions and plan of care coordination, and collaborate closely with an interdisciplinary team to ensure quality outcomes. Requirements

• Clear and active Texas LVN license
• Medicare home health experience
• Familiarity with OASIS documentation
• Strong clinical and organizational skills
• Value collaboration and patient-centered home care

Responsibilities

• Managing a growing patient census
• Providing direct nursing care
