About The Position

The Home Health Licensed Practical Nurse (LPN) is responsible for providing home health nursing care to patients according to a physician's established plan of care. This role operates under the direction of a registered nurse and must comply with state Nurse Practice Act regulations and company policies. The position requires flexibility, the ability to manage stress, and excellent communication skills. The LPN will be responsible for direct patient care, documentation, and collaboration with healthcare team members.

Requirements

Currently licensed as a Licensed Practical Nurse under the laws of the state of Oklahoma.
Must be flexible and able to adjust to changing tasks frequently.
Must have valid driver's license, reliable vehicle, maintain valid auto liability insurance, and be able to drive throughout service area under a variety of weather conditions.
Must be able to maintain concentration during frequent interruptions.
Excellent verbal and written communication skills.

Nice To Haves

Previous home health nursing experience preferred.
Recent experience in direct patient care in an institution or home health setting is preferred.
Light to moderate degrees of stress incurred in daily employment.

Responsibilities

Delivers visits/service to assigned patients and performs direct care in accordance with physician-ordered plan of care to provide optimal care.
Assists DCS in ongoing observation and care of assigned patients.
Maintains communication regarding patient progress with physician, supervisor, staff, and family members as appropriate.
Documents all nursing observations, interventions, and evaluations in a concise, accurate, and timely manner.
Reports patient-related observations to DCS daily or as needed.
Participates in on-call, holiday, and weekend rotation as assigned.
Participates in evaluation of patients in preparation for discharge.
Uses supplies appropriately and demonstrates a cost-conscious awareness.
Maintains equipment and supplies in proper functioning order and reports any malfunction immediately.
Demonstrates willingness to assist co-workers whenever needed.
Maintains a good working rapport with all patient care personnel and other departments.
Participates in teaching patients, family members, and/or significant others as appropriate.
Refers to other members of the health care team to meet specific learning needs of the patient.
Notifies DCS/Administrator of changes in patient condition as necessary.
Addresses patients, families, and co-workers in a responsive and respectful manner.
Always delivers safe care that is in concert within physician orders, home health policy and established nursing care standards.
Attends and contributes to both formal and informal patient care planning conferences.
Completes all forms concerning patient visits and mileage information according to company policy.
Communicates new ideas and suggestions for company operation and performance.
Maintains and respects patient confidentiality.
Understands the patient's medical record is a legal document and maintains accurate documentation and respects confidentiality of same.
Attends ongoing infection control education programs plus a minimum of 95% of staff meetings per year.
Reads and initials staff meeting minutes if unable to attend such.
Improves patient care through participation in continuing education and in-service.
Complies with Progressive’s emergency preparedness plan.
Provides accurate, timely, complete, and acceptable documentation.
Exhibits sound judgment and technical skills necessary to perform assigned nursing treatments and procedures.
Effectively communicates with team members to keep them informed of changes in patient condition and needs.
Notifies DCS and Administrator immediately whenever unable to complete patient care assignment.
Demonstrates awareness through safe practice of established nursing policies and procedures related to every technical procedure performed.
Demonstrates knowledge of underlying disease processes, surgical procedures, or injuries of assigned home care client/patient.
Displays adequate knowledge of safe administration of medications and the instruction of patients.
Implements care by setting priorities based on the client/patient care plan and immediate patient need.
Evaluates the response of patients to nursing interventions and revises care plans with DCS as necessary.
Performs all treatments and procedures with consideration for the patient's condition, safety, comfort and privacy.
Notes normal and abnormal conditions and reports accurately.
Demonstrates awareness of infection control policies and procedures and the practice of asepsis in the performance of all personal patient care and treatment procedures.
Observes universal precautions when caring for all clients/patients.
Practices isolation techniques according to established company policies and procedures.
Complies with OSHA regulations and recommendations.
Practices good body mechanics when lifting or transferring clients/patients or equipment.
